Contracting with ASNCM

PURCHASED SERVICES REQUEST FOR PROPOSALS (RFP)

The Executive Office of Elder Affairs (EOEA) contracts with Aging Services Access Points (ASAPS) throughout the Commonwealth of Massachusetts, to administer homecare programs for persons sixty years of age and over. Aging Services of North Central Massachusetts (ASNCM) is a private, non-profit agency, serving as the ASAP in 21 cities and towns in North Central Massachusetts.

ASNCM negotiates rates within the Executive Office of Health and Human Services (EOHHS) and EOEA guidelines, and reserves the right to accept or reject any proposal. Before issuing a provider agreement, ASNCM must receive the following documents:  a copy of your certificate of insurance, Form W-9, a completed Administrative Overview, completed Service Specific Attachment(s) for all services you wish to provide, and all other associated documentation requested in this application.

In-Home Services Provider Survey – Homemaking, Personal Care, Home Health Aide, Skilled Nursing (Medicare Certified Services), Supportive Home Care Aide, Companion, Chore, Laundry Services, Physical Therapy, Speech Therapy, Home Safety/Independence Evaluations (Occupational Therapy), Habilitation Therapy, Vision Rehab, and Orientation and Mobility.

Non-In-Home Services Provider Survey – Adult Day Health, Transportation, Translation, Alzheimer Coaching, Emergency Shelter, Respite, Supportive Day Program, Behavioral Health Services, Evidence Based Programs, Goal Engagement Program, Peer Supports, Personal Emergency Response System, Enviornmental Accessibility Adaption, Transportation, Translation, Durable Medical Equipment, Wanderer Locator, Medication Dispensing System, Virtual Communiation and Monitoring, and Home Delivered Meals.
