The Community Resources Manager position provides leadership in several programs that are crucial for our mission.

Our Information & Referral program is a door opener for anyone who contacts us as we connect people to individuals on a wide-range of community-based services and programs.  Through our Options Counseling program we give one on one support to individuals and their families so that they can make informed decisions.  Our Caregiver program provides support and services to empower balance and well-being for the caregiver.  Our new Care Transitions Liaison Program is a gateway of hope that someone in a nursing facility can and will return to their home, to their community.

Amazing when you understand the objective of each of these programs – what a positive difference we get to make in the lives of older adults.

What is even more amazing is our Community Resources Manager, Cynthia Sans.  Cynthia has been in this role for over a month, and I can profoundly attest that her experience, professionalism, and drive is exceptional.

Cynthia has been a Geriatric Support Specialist since October 2021.  She has a strong background Quality Improvement and has proven development and implementation of Results Orientate Management Plans.  Cynthia knows data, she knows people and she knows how to build and improve processes through collaboration.   Cynthia received her Bachelor of Arts in History and English (double major) from Mount Holyoke and has an Associate’s Degree in Liberal Arts and Sciences from Mount Wachusett Community College.

Cynthia is an eXtraordinary Gen X-er!  Her leadership style – is S – Steadiness.  This style is considered even-tempered, accommodating, patient, humble and tactful.  She also has a subset style of I – Influence.  This style is outgoing, enthusiastic, and optimistic.
